File a Product Claim

Use this form to address your concern reguarding an existing order, and get a fast response.

Here is How to Properly File a Claim

Please note, customers must inspect product upon arrival to their facility. FMI Farms will only honor claims made within 48 hours of receiving your order.
Please follow the instructions below to successfully process your claim.

1. Take a Photo of Wrapper

This step may not be applicable for flowers
that don't come in a wrapper.

2. Take a Photo of Barcode

This step may not be applicable for flowers
that don't come in a wrapper.

3. Take a Photo of Entire Bunch

Take a clear picture of the entire bunch
showing the damaged stems.

4. Take a Photo of All Damaged Stems Together

Take a aerial picture of all the the damaged or bad stems together. Lay them out so the individual stems are clearly visible.

5. Take a Photo of Each Damaged Stem

Take individual close up pictures of every damaged or bad stem being claimed as demonstrated in the images below.

6. Fill out Claims Form

Once you are done collecting photographic evidence of damaged or bad items. Fill out the claims form, attached images and submit your claim. Upon validation of customer’s claim, a credit memo will be issued and sent to customer viaemail, fax, or mail.
